I request to be admitted to the "Eclisse Club APS" Association based in MONZA, Via Borgazzi, 190, as a member.

By signing this application, you declare that you elect your domicile, for the purposes of associative life, at the headquarters of the Association, and undertake to accept and observe, and have family members and any guests observe in full, the statute and the internal regulation which it approves and which it declares to have read.

The sum paid as a contribution is to be understood as an advance of the membership fee and partial reimbursement for secretarial, management and promotion expenses, as well as any voluntary contributions in cash or assets that may be paid to the Association later.

I expressly approve the clauses of the statute regarding the purpose, the admission and exclusion of members, the election of domicile (for the purpose of associative life), the active and passive electorate, the powers of the board of directors and the arbitration clause.
